This app is for legal aid organizations looking for standard templates that request for a client’s medical records from the Social Security Administration to aid in the appeal process for Social Security disability benefits. The form included in this workflow is the Letter to Client template. It is important to note that these templates include Minnesota states statutes. Unless you are a legal aid organization in Minnesota, these templates must be updated to reflect your state’s statutes. This is only possible thanks to the generous funding by Legal Services Corporation through the Technology Innovation Grant.
